The “Healthy Me” prompt gives space to jot down practical ways to keep well across thinking, exercise, relaxation, social support, professional support and enjoyable activities. It is a concise staying healthy and relapse prevention handout for people maintaining progress after depression treatment, counselling, CBT or medication.
Staying healthy after depression treatment often involves noticing what supports day-to-day wellbeing and having a clear plan for times when mood starts to dip. This printable PDF sheet helps people summarise the habits, supports and activities that can help them maintain progress after counselling, CBT, medication or other depression treatment.
The handout can be used near the end of therapy, during relapse prevention planning, in a follow-up appointment, or as part of a personal wellbeing folder. It prompts the person to think about practical areas such as helpful thinking, exercise, relaxation, enjoyable activities, social contact and professional support. It also encourages a balanced view of normal setbacks, so low days can be planned for without being treated as failure.
It may be useful for adults and older teenagers working on depression recovery, low mood management or mental health maintenance. Practitioners such as counsellors, psychologists, social workers, wellbeing workers and GPs can use it as a simple worksheet for after-care planning, review sessions or guided self-help.
Download and print the sheet to complete in a session, add to a recovery plan, or keep somewhere accessible as a reminder of the routines and supports that help with staying well. Prepared by the Centre for Clinical Interventions.
